Instructions for completing forms:
- Download and print the required medical forms.
- NEW STUDENTS: Commonwealth of Virginia Entrance Health Form
- ALL STUDENTS (forms on OakcrestConnect):
1. Emergency Permission to Treat2. Medical Form Uploads3. OTC Medication Authorization
- ALL STUDENT-ATHLETES: VISAA Health Form
- Fill out each form and have them signed by your daughter's primary care physician.
- Take a photo or scan completed forms to create a PDF.
- Upload the photo or PDF to OakcrestConnect.

Vaccines required for 7th and 12th Grade Students:
Rising 7th and 12th graders: Please submit the most recent immunization record.
Rising 7th and 12th graders: Please submit the most recent immunization record.
Rising 7th graders: A booster dose of Tdap is required for all students entering the 7th grade.
Rising 12th graders: A final dose of the meningococcal conjugate vaccine (MenACWY) is required prior to entering 12th grade. If your daughter received the first dose after age 16, a second dose is not required. Please submit documentation if she received the first dose after age 16.
